Placing of the deceased into our care when a death takes place in New York City (Including The Bronx, Brooklyn, Queens, Manhattan, Staten Island or Westchester County, and safe return to Our Facility.

Providing safety and shelter of the body in our Funeral Home, in a temperature controlled enviornment

Basic Services of Funeral Director and Staff including 24-Hour availability, providing the party responsible for the deceased with pricing disclosures and authorization forms necessary for cremation.

Obtaining a Certificate of Death signed by the Deceased's Attending Physician, person acting as such, or the Medical Examiner

Completing the Certificate of Death with information provided to us, by a responsible party, and submitting the form to the NYC Dept. of Health and Mental Hygiene, or Registrar of Vital Statistics

Obtaining permission from the NYC Office of the Chief Medical Examiner to cremate, if applicable

Providing a Cardboard Alternative Container in which the body is placed into at our facility, prior to being shuttled to the crematory

Transit of the body, in the alternative container, from our Funeral Home to the Crematory Facility

Only payment to City Funeral Service is required. For your convenience, we accept all forms of Major Credit Card, or Debit.
City Funeral Service prefers to engage with The Historic Woodlawn Cemetery in The Bronx as our Crematory of preference. Woodlawn will perform this delicate process, and the fee listed here includes forwarding of the ashes to you or designated party using the US Postal Service. If you prefer we use another Crematory, please tell us when finalizing services so that we may quote you the correct price. All Souls Crematory (East Elmhurst, Queens), Fresh Pond Crematory (Middle Village, Queens), Ferncliff Cemetery (Westchester) and Greenwood Cemetery (Brooklyn) are reputable organizations in New York offering Cremation to Funeral Directors.

Beyond New York City, on Long Island or in Westchester, Rockland and Putnam County, this document is issued by the Registrar of Vital Statistics in the City, Town or Village where the death ocurred. This document will accompany the Deceased to the Crematory

Certified Copies of the Certificate of Death bear the Seal of the City of New York, or Municipality where the death has taken place. The copies are used for Insurance Claims, Probate, Property Transfer, or Banking and Financial Transactions. The number of copies needed is determined by the size of the Deceased's Estate. You do not have to order any, but we recommend you order one for your records. The City of New York will provide three (3) copies at no charge if the Deceased is an Honorably Discharged Veteran of the US Armed Forces. However, please keep in mind City Funeral Service DOES NOT issue any copies free, nor are we a registrar. We merely order the copies for you on your behalf, as a convenience to you

Not too long ago, Funeral Directors obtained a Certificate of Death from a Physician or Medical Examiner on a Handwritten Paper Form. We then submitted this to the Department of Health, in person, at their office. As New York City Government finally enters the 21st Century, Funeral Directors and Physicians must use eVital, an internet based system for completing and submitting Certificates of Death to the NYC DOHMH. The fee for system utilization is listed to the right. The customer will receive the Certified Copies on special paper from NYC DOHMH
